This is a comprehensive practical guide to the practice of the new subspecialty of critical care echocardiography. The text covers all aspects of clinical practice and describes how to use transthoracic echocardiography to approach and manage common clinical questions. The text covers assessment of, and disorders affecting, the left and right heart, including shock and sepsis. It also looks at how to interpret diastolic information during critical illness and the effect of that illness on valve disease and function. The important subject of fluid responsiveness is explored in detail, as is the influence of organ support and illness on the diagnosis of cardiac tamponade. The information given in the text is summarized using algorithms in a field guide for the critical care echocardiographer, which also provides a rapid guide to assessing time-critical patient presentations. Integrating advanced planning, strong teamwork, A systemic review was performed to evaluate the epidemiological, pathophysiological, and clinical features of sarcopenia, the relationship of sarcopenia with critical illness and its impact on mortality, and diagnostic methods Critical emergency medicine is the medical field concerned with management of critically ill patients in the emergency department ED. Background: Despite the importance of microcirculation in organ function, monitoring microcirculation is not a routine practice.

A decade of progress in critical care echocardiography: a narrative review

Metrics details. Urgent cardiac ultrasound examination in the critical care setting is clinically useful. Application of goal-directed echocardiography in this setting is quite distinct from typical exploratory diagnostic comprehensive echocardiography, because the urgent critical care setting mandates a goal-directed approach. Accordingly, this paper highlights 1 indications, 2 an easily memorized differential diagnostic framework for goal-directed echocardiography, 3 clinical questions that must be asked and answered, 4 practical issues to allow optimal image capture, 5 primary echocardiographic views, 6 key issues addressed in each view, and 7 interpretation of findings within the differential diagnostic framework. The most frequent indications for goal-directed echocardiography include 1 the spectrum of hemodynamic instability, shock, and pulseless electrical activity arrest and 2 acute respiratory failure. RESP-F for exacerbation of chronic Respiratory disease, pulmonary Embolism, ST changes associated with cardiac or pericardial disease, Pneumonia, and heart Failure can be used for acute respiratory failure. The goals of goal-directed echocardiography in the unstable patient are: assessing global ventricular systolic function, identifying marked right ventricular and left ventricular enlargement, assessing intravascular volume, and the presence of a pericardial effusion.

There has been a recent explosion of education and training in echocardiography in the specialties of anesthesiology and critical care. These devices, by their impact on clinical management, are changing the way surgery is performed and critical care is delivered. A number of international bodies have made recommendations for training and developed examinations and accreditations. The challenge to medical educators in this area is to deliver the training needed to achieve competence into already overstretched curricula.

Metrics details. The echocardiography working group of the European Society of Intensive Care Medicine recognized the need to provide structured guidance for future CCE research methodology and reporting based on a systematic appraisal of the current literature. Here is reported this systematic appraisal.

Critical Care Statistics

This narrative review focusing on critical care echocardiography CCE has been written by a group of experts in the field, with the aim of outlining the state of the art in CCE in the 10 years after its official recognition and definition. Its use, both as a diagnostic tool and for hemodynamic monitoring, has increased markedly, influencing contemporary cardiorespiratory management. Recent studies suggest that the use of CCE may have a positive impact on outcomes.

Bedside use of Doppler echocardiography is a promising, invaluable clinically useful instrument to manage critically ill patients. It provides rapid, comprehensive data about the cardiovascular system [1, 2]. Training intensivists on echocardiography still represent a challenge.

Echocardiography allows a real-time, non-invasive and radiation-free bedside evaluation of cardiac structure and function. Indeed, echocardiography provides clinically relevant information not only about myocardial systolic and diastolic function, cardiac valves, and pericardium but also about volume status and pulmonary congestion. However, most critical care echocardiograms are more a complement to physical examination and invasive monitoring rather than systematic, complete studies. For appropriate and reliable acquisition and interpretation of echocardiographic studies, physicians performing echocardiograms should have been adequately trained. In addition to a sufficient number of supervised exams, physicians should have solid knowledge of cardiac anatomy, physiology, pathophysiology, and principles of ultrasonography. The Acute and Critical Care Echocardiography book, edited by Claire Colebourn and Jim Newton, provides an overview of the possible applications of echocardiography in acute and critical care medicine.

The echocardiography has become a vital tool in the diagnosis of critically ill patients. This tool has become a common procedure for the cardiovascular assessment of critically ill patients, especially because it is non-invasive and can be applied in fast and guided manner at the bedside. Physicians with basic training in echocardiography, both from intensive care unit or emergency department, can assess the left ventricle function properly with good accuracy compared with assessment made by cardiologists. The change of treatment approach based on echocardiographic findings is commonly seen after examination of unstable patient. This brief review focuses on growing importance of echocardiography as an useful tool for management of critically ill patients in the intensive care setting along with the cardiac output assessment using this resource.
